help with cris forms

CRIS forms may be obtained on the web at the CRIS Forms Assistance Homepage.

There are two way to prepare CRIS forms:

Print Blank Forms
From the CRIS Forms Assistance Homepage click on "CRIS forms" and then choose the second option: "Print Blank Forms". This will print any of the required forms in Adobe Acrobat, resulting in a hard copy of the form which must then be completed manually.

Prepare CRIS Forms on the Web
Another option is to enter the information on-line. This process begins at the CRIS Forms Assistance Homepage. Click on the grey section titled "Enter Forms" under the heading “Work Unit Description (AD-416 / AD-417 / 2008).”  On the picture of the US map, click on California. The site for the UCD School of Veterinary Medicine is "CALV."  The password is ‘sundial.’

Type in the PI's last name. If this is a new project, check the box "New Formula Funded or State Project". Follow the instructions.
