CRIS Forms on the Web

CRIS forms may be obtained on the web at the CRIS Forms Assistance Homepage, http://cwf.uvm.edu/cris/.

To Print Blank Forms:

From the CRIS Forms Assistance Homepage at http://cwf.uvm.edu/cris/ click on "CRIS forms" and then choose the second option: "Print Blank Forms". This will print any of the required forms in Adobe Acrobat, resulting in a hard copy of the form which must then be completed manually.

To Prepare CRIS Forms on the Web:

From the Homepage at http://cwf.uvm.edu/cris/ click on the light blue section for Investigators titled "AD-416/417." On the picture of the US map, click on California. The site for the UCD School of Veterinary Medicine is "CALV." You cannot enter this site without a password. Please contact your Department MSO or Bonnie London at brlondon@ucdavis.edu to obtain the password.

Type in the PI's last name. If this is a new project, check the box "New Formula Funded or State Project". Follow the instructions.
