1. Download the diagnostics submission form below (the form includes shipping instructions, what sample type to submit, prices and panels).
SUBMISSION FORMS/DIAGNOSTIC TESTS AND PANELS
2. Print PAGE 2, fill it out completely and include with your sample. Please remember to write down the test you are requesting, and where we should fax results.
3. Make sure all your samples are in thoroughly sealed tubes or containers. Example: drippy poop in a glove is not a sealed container.
4. Ship the samples OVERNIGHT to the following address.
