Shipping Permits

Canada (canine and feline samples only)
The Lucy Whittier Molecular & Diagnostic Core Facility can receive samples from dogs and cats originating from Canada.  Please print the instructions and the Statement about Sample.  The Statement about Sample must be on your facilities letterhead.  Just fill out the Statement about Sample and attach to the outside of the package. For equine samples please use a general Import Permit.

The Lucy Whittier Molecular & Diagnostic Core Facility is happy to announce that we can now accept samples from other countries.  Please print our Import Permit, fill out the Statement of Health letter and attach to the outside of the package.
