Case 8

mmode.jpg (31617 bytes)

This M-mode echocardiogram shows several things. The left ventricular (LV) cavity is enlarged as seen in the previous two-dimensional echocardiogram to approximately 21 mm in diastole. The motion of the left ventricular free wall (LVFW) is vigorous. Still the end-systolic diameter is approximately 12 mm, which is increased. The increase in end-systolic diameter in this case is probably due to a decrease in left ventricular contractility (myocardial failure). The right ventricular free wall (RVFW) is severely thickened. The interventricular septum (IVS) moves abnormally. When examined on the two-dimensional echocardiogram it could be appreciated that this abnormal motion was because the interventricular septum flattened in systole and became round again in diastole.
