Prerequisites Substitution Request Form

Before submitting a substitution request form, please review the institutions listed in the Prerequisites Database for pre-approved transferable courses. The majority of California instititions and a limited number of out-of-state colleges are listed here. Prerequisites taken at California public colleges are articulated on the ASSIST.org website and will not be substituted. 

Upper division courses taken at California private colleges or out-of-state colleges may be reviewed for approval to satisfy science prerequisites. Generally, lower division courses do not need to be submitted for prerequisite approval. Statistics does not require approval. Any course, lower or upper division, in statistics will suffice. (Calculus is not required and will not satisfy the statistics requirement). 

Please submit one form per course and include all required information. Incomplete forms may not be evaluated.
